چکیده
A 29-year-old woman without known medical illness was evaluated for incessant palpitations. She presented to an emergency department multiple times, where various diagnoses were made, including junctional tachycardia, atrial tachycardia, and atrial fibrillation. Therapy with propafenone, metoprolol, and rivaroxaban was initiated, but her symptoms continued. Echocardiography did not show evidence of structural heart disease. The patient was then referred to our hospital for further treatment and potential ablation for atrial fibrillation. At her clinic visit, a 12ECG revealed an irregular narrow complex tachycardia with group beating pattern (Figure 1). What is the rhythm shown in Figure 1? Please turn the page to read the diagnosis. An irregular, narrow QRS complex tachycardia with ventriculoatrial block: what is the tachycardia mechanism?
A 57-year-old woman underwent an electrophysiology procedure because of a 19-year history of recurrent palpitations. Typical AV nodal reentrant tachycardia that had a cycle length of 460 msec was induced by ventricular pacing. Slow pathway ablation was attempted using a combined anatomical and electrogramguided approach. After an application of radiofrequency energy associated with junctional e...
متن کاملImages and Case Reports in Arrhythmia and Electrophysiology Wenckebach During Supraventricular Tachycardia
In case 1, a 71-year-old man with a history of coronary artery disease presented with recurrent paroxysmal supraventricular tachycardia (SVT) despite treatment with -blockers. During electrophysiology study, the baseline sinus cycle length was 895 ms, atrial-His (AH) interval was 120 ms, and His-ventricular interval was 60 ms. A narrow complex SVT with tachycardia cycle length of 480 ms was ini...

A 66-year-old woman with a history of hypertension and persistent atrial fibrillation was referred to the Arrhythmia Clinic. Before her visit, a routine 12-lead ECG was ordered (Figure 1). The patient was asymptomatic except for minor palpitations. Her medicine regimen included apixaban, flecainide 150 mg twice daily, lisinopril, loratadine, metoprolol, and simvastatin. What is the cardiac rhyt...
متن کاملTwo simultaneous right ventricular tachycardias in a case of arrhythmogenic right ventricular dysplasia.
A 51 year old woman with arrhythmogenic right ventricular dysplasia had two types of ventricular tachycardia--(a) a regular and sustained tachycardia and with normal frontal plane axis on electrocardiography and (b) an irregular non-sustained tachycardia with a leftward frontal plane axis. Changes in the QRS complex were sometimes seen during the sustained ventricular tachycardia. The clinical,...
متن کاملRegular narrow QRS and regular wide QRS tachycardias in a woman with mitral regurgitation of uncertain etiology.
A 64-year-old woman had been experiencing short episodes of rapid regular heart beating for nearly a year. The bouts became frequent and lengthy, resulting in a hospital admission a month earlier when intravenously administered adenosine terminated an episode. She was discharged on diltiazem, but the bouts of tachycardia continued and were accompanied by light-headedness and sometimes nausea. W...
